Back-to-School Forms
Skyward
Every LCS family must have an account in Skyward, our student information system. This system is used to complete forms, check account balances, view MS/HS schedules, access MS/HS gradebooks, and more. To successfully create an account, you must complete all steps as directed below. Please note: Skyward often uses “pop-ups” in its processes, so please turn your pop-up blocker off in order to complete these steps.

Back-to-School Forms
All families must complete annual back-to-school forms electronically through Skyward before the first day of school. Directions:
- Visit the LCS Skyward Website
- Click “Family Access Login”
- Use your ID and Password to log in to Skyward
- Click “Back-to-School Forms” or “Go to Back-to-School Forms for (Student)”
- Proceed step by step through the verification process
- When finished, click “Submit Back-to-School Forms” to complete the process

Elementary (K-5) Forms

- Background Check Form
- FERPA – (Optional) The Michigan Department of Health requires that LCS obtains permission on the Consent for Disclosure of Immunization Information to Local and State Health Departments from parents/guardians of any new student. If you do not wish to disclose your child’s immunization information to the local and state Health Departments, please email the elementary office saying that you do not want to disclose that information.
Middle and High School (6-12) Forms

- Vaccination form
- Please email your up-to-date immunization record from your doctor’s office to the middle or high school office.
- Athletics
- FERPA – The Michigan Department of Health requires that LCS obtains permission on the Consent for Disclosure of Immunization Information to Local and State Health Departments from parents/guardians of any new student. If you do not wish to disclose your child’s immunization information to the local and state Health Departments, please email the middle or high school office saying that you do not want to disclose that information.
