Employees that are considered 50%FTE or work over 1,000 hours a year qualify for $50,000.00 worth of basic life/AD&D coverage for yourself and family coverage, which includes $10,000 worth of coverage for your spouse and/or $2,000 worth of coverage for each of your dependent children.
If you would like more life insurance or AD&D coverage for yourself or your family, you may fill out the ‘Opt Life Application’ and return it to the CSI Insurance office within 30 days of your benefits effective date and the additional coverage will be automatically approved. To add coverage more than 30 days after your benefits effective date, your application will need to go through an underwriting process and could be denied. You may also opt additional life insurance during open enrollment.
Your benefits also include short term disability coverage that will pay 75% of your pre-disability salary on a weekly basis following a 4 week waiting period should you become disabled. Benefits are payable for up to 5 months.
Long term disability benefits are also included and will pay 60% of your pre-disability salary on a monthly basis following a 6 month waiting period should you become disabled. An added benefit of the CSI long term disability benefits is the Ability Assist counseling services available through The Hartford. Please see the attached flyer for more information on what is included and how to access the services.
You may add optional vision coverage if you would like. The vision enrollment form is linked below along with a flyer outlining the 2 different vision plans that are available. Employees who become eligible for benefits part way through the plan year (September 1 – August 31) can enroll in vision benefits only if the equivalent of 12 months of premium is paid within the plan year. As an added benefit, members who are enrolled in the VSP vision insurance also have access to a hearing aid discount.
You have the option to add critical illness, hospital indemnity, or accident insurance to your coverage.
Here is a guide to how long your dependent children are eligible for each of the benefits.
· Life – Through the end of the month in which the unmarried dependent turns 26 or unmarried children of any age if they are permanently and totally disabled.
· Vision – Through the end of the month in which the dependent turns 26, regardless of student or marital status; or unmarried children of any age if they are permanently and totally disabled.
Please make sure you give your completed enrollments form(s) to the school office within 30 days of your hire date or complete the information online during open enrollment.
We also offer a Student Loan Assistance benefit that all employees who are eligible for benefits are enrolled in. This benefit includes student loan coaching, student loan strategy, and guidance to achieve Public Student Loan Forgiveness (PSLF). Your family members will also be eligible to use all of these services except the PSLF guidance. These benefits could potentially save you thousands of dollars on your student loans and there is no cost for your school or for you to take advantage of them. You will see more detailed information about this benefit in your insurance welcome packet.
